Joint Injections

A joint is any area of the body where two bones connect. Due to injury or disease, the space between the two bones can sometimes become swollen and inflamed, which leads to pain and a loss of mobility. Injection and aspiration are two tools that physicians use to treat joint pain locally without needing to perform surgery.

Both injection and aspiration are techniques that involve inserting a needle connected to a syringe directly into the joint. Aspiration involves the removal of excess fluid, and injection is the placement of medication directly into the joint space via the needle. Both procedures may be performed at the same time. In both cases a local anesthetic may be used, the skin will be cleaned and disinfected, and then the needle will be inserted. In some cases, especially if the injection or aspiration is in a large and deep joint such as the hip or spine, ultrasound may be used to guide the needle to the exact location desired.

In some cases, irritation to the joint can cause fluid to build up so significantly that the joint hurts and can no longer move well. Aspiration removes some of the excess fluid and relieves the pressure. In addition, aspiration can be used to provide a sample of joint fluid if it needs to be examined microscopically for the presence of white blood cells, bacteria, or crystal formations.

Certain injuries and diseases that affect joints are inflammatory in nature, such as rheumatoid arthritis, tendonitis or gout. These diseases may be helped by the local injection of anti-inflammatory medications directly into the affected joint. In this case, corticosteroids such as methylprednisone are usually used. Relief from the pain may be felt right away and may last for weeks or even months.

In some cases, such as with osteoarthritis, the cartilage buffer between the two bones wears down and pain comes from bones rubbing against each other. In this case, injection of a lubricating agent such as hyaluronic acid may be beneficial. It provides a slippery cushion between the bones to relieve pain that can last for months.

This was a consultation visit for pain in my foot. My PCP referred me here as I asked to go back. In 2017, I had a bunionectomy performed by this office. Brad remembered me and, while in the visit, pulled up my x-rays from that surgery. I appreciated that he had those available, and we could see what had changed in the years since first seeing them. I have a lot of respect for Brad going over treatments and listening to me when I said I had tried many of them. He didn't even suggest we try them again like many providers. He took my concerns seriously when I stated one made the pain worse, and the other made me more unstable, and I almost fell multiple times. I am glad I asked to come back and that the same staff are here. The continuity of care was excellent, and I have faith in the treatment plan and recommendations they give me.

PA Brad J. Bachmeier provided outstanding care. He truly listened to my concerns, took the time to explain things clearly, and demonstrated an exceptional level of knowledge and professionalism. After having subpar experiences with nurses and some members of the care team at the surgery center, Brad stood out in every way. His service was above and beyond, and he made a stressful experience feel manageable and reassuring. I'm very grateful for the care he provided and would highly recommend him. Out of everyone I saw for my surgery, Brad was by far the most knowledgeable and compassionate!

I have a broken wrist. Twice I called with concerns re: a painful problem, then looseness with the splint, then cast. The providers were good, but I definitely waited til the end of the day to get a call back from a call placed early morning. I felt that the Issue was somewhat "urgent." Overall, good care, but I did experiences delays due to scheduling & "lack of someone available" to take care of that today- and going to an Urgent care or ER would not have been a helpful solution. I do know there are staffing issues- and everyone was doing the best they could

Provider was personable, helpful, clarifying the extent of my injury and how to care for my self over my recovery period. I have worked with a lot of orthopedic doctors . This one was the most personable, engaged and communicated the degree of my injury and cautioned me about overdoing and what the consequences would be. All done with care. Please note this was a follow up for an urgent care visit for a broken foot which happened 7 days prior to my visit with this specialist. I believe it should have been scheduled sooner.

Brad is the best PA that I see throughout the entire Providence system. He has helped me several times and I have the utmost respect and confidence for the assistance he has given me. Saw Brad as a referral for chronic foot pain. He did X-rays, identified my issue, made recommendations. I really appreciated that he encouraged me to stay as active as I am (I walk 4 or 5 miles daily) and recommended where I could find orthotic help that would not break the bank for me as I'm retired. Also he agreed with me that the least / lowest-impact resolution is the best place to start. He was very easy to talk too and made me feel that my issue does matter. Great guy!

I was very pleased with how thorough, kind and informative the provider was! I learned a lot that I did not previously know. He took all the time needed to help me. Additionally, when we went downstairs to get the boot I needed, the person there was initially indicating that we couldn't be helped that day. As the PA for my surgeon, he Is extremely competent and I was very satisfied with each of my appointments with him.
